DS-11 Passport Application Instructions: US Passports for Children

A: Parents must complete and print two copies of the:
DS-11 Passport Application Form for Children (minors under 16).

Your Child's DS-11 Passport Application must:

  • not be signed until instructed to do so during the passport acceptance process.
  • be completed using the Online DS-11 Passport Application Form through the Passport Agency link above. Hand filled applications will not be accepted.
  • have original dates and signatures and written be in black ink. Copies of signatures and dates or those in colored ink or pencil are not acceptable.
  • be printed on two separate pages. Applications printed on the front and back of a single sheet are not acceptable.
  • be submitted to the Passport Agency within 5 business days of the date your documents are executed and sealed.
Proof of Citizenship Requirements: US Passports for Children

Q: What do I need to provide to prove my child's status as a citizen of the United States to get my child a US Passport?

A: The US Department of State Passport Agency requires one of the following documents as evidence that your child is a legal citizen of the United States:

  • A Birth Certificate showing birth in the US: This must be original or certified copy long-form certificate showing your parents names and issued by the municipality in which you were born; OR
  • A Previous US Passport: The passport can be either valid or expired. If your appearance has changed significantly since this passport was issued, you may need to submit a secondary proof of US citizenship; OR 
  • A US Naturalization Certificate – This must be original or certified copy.

Please Note: This document will be returned to you with your Child's new passport.

Photo Requirements: US Passports for Children

A: You will need to provide two passport photo's of the child which should be:

  • in color & not blurry.
  • 2x2 in size
  • front view with body completely facing forward.
  • on a plain white background.
  • with no glasses or head wear.
  • not older than 90 days
  • taken at a reputable facility (Walgreen’s, CVS, or Fed Ex).

 

Proof of Travel Requirements: US Passports for Children

A: A copy of your child's flight itinerary or flight booking confirmation email.
If your child is accompanying you while traveling on business and no itinerary is available, you may instead provide a business letter on company letterhead.

Important Note: Your proof of travel must show departure within the next 15 business days or 21 business days if a visa is needed.

Passport Agency Authorization Letter Instructions: US Passports for Children

A: The Passport Agency Letter of Authorization allows UVC to:

  • submit your Child's passport application to the passport agency for expedited processing;
  • speak and interact with the Passport Agency as an agent on your behalf;
  • pick up your child's new or updated US Passport and ship it to you.

Passport Acceptance Facility Instructions: US Passports for Children

A: Passport Acceptance Facility is authorized by the U.S. Department of State Passport Agency to accept and verify the accuracy of passport documents. This verification process of passport documents is called Passport Execution. 

Before proceeding, make copies of all required passport documents to include in your shipment to UVC;
OR
email or upload copies of all required passport documents to UVC for pre-ship review.
This is mandatory for document verification per US Department of State Passport Agency Regulations!

 

  1. Both parents must appear with the child with all required passport documents at a Passport Acceptance Facility. To locate your nearest Passport Acceptance Facility, enter your zip code in the Passport Acceptance Facility Locator. 

  2. Present your child's documents to the Passport Acceptance Facility Agent and inform the agent you will be using an expediting service to “hand carry & expedite” your DS-11 Child Passport Application. They will charge you a 35.00 service fee.

  3. Once both parent's present valid ID to prove your identity, the agent will review your child's documents, have both parents sign the DS-11 child passport application, then package everything into an envelope which will be sealed and stamped with a federal markings.

  4. The sealed passport application envelope will be handed back to you for expedited processing and that completes the Passport Execution Process. This envelope must be submitted to the US Department of State Passport Agency within 5 business days or it becomes invalid so ensure to overnight ship US Visa Connection right away.
