DS-11 Passport Application Form for Replacing Lost/Stolen US Passports.

Your DS-11 New/First Time Passport Application must:

  • not be signed until instructed to do so during the passport acceptance process.
  • be completed using the Online DS-11 Passport Application Form through the Passport Agency link above. Hand filled applications will not be accepted.
  • have original dates and signatures and written be in black ink. Copies of signatures and dates or those in colored ink or pencil are not acceptable.
  • be printed on two separate pages. Applications printed on the front and back of a single sheet are not acceptable.
  • be submitted to the Passport Agency within 5 business days of the date your documents are executed and sealed.
Proof of Citizenship Requirements: Replacing a Lost/Stolen US Passport

A: The US Department of State Passport Agency requires one of the following documents as evidence that you are a legal citizen of the United States:

  • A Previous US Passport: The passport can be either valid or expired. If your appearance has changed significantly since this passport was issued, you may need to submit a secondary proof of US citizenship; OR 
  • A US Naturalization Certificate – This must be original or certified copy.

  • in color & not blurry;

Proof of Travel Requirements: Replacing a Lost/Stolen US Passport

Passport Acceptance Facility Instructions: Replacing a Lost/Stolen US Passport

A: Passport Acceptance Facility is authorized by the U.S. Department of State Passport Agency to accept and verify the accuracy of passport documents. This verification process of passport documents is called Passport Execution. 

Before proceeding, make copies of all required passport documents to include in your shipment to UVC;
OR
email or upload copies of all required passport documents to UVC for pre-ship review.
This is mandatory for document verification per US Department of State Passport Agency Regulations!

 

  1. Take all required passport documents to your nearest Passport Acceptance Facility. To locate your nearest Passport Acceptance Facility, enter your zip code in the Passport Acceptance Facility Locator. 

  2. Present your documents to the Passport Acceptance Facility Agent and inform the agent you will be using an expediting service to “hand carry & expedite” your DS-11 Lost/Stolen US Passport Replacement Application. They will charge you a 35.00 service fee.

  3. Once you present your valid ID to prove your identity, the agent will review your documents, have you sign the DS-11 passport application, then package everything into an envelope which will be sealed and stamped with a federal markings.

  4. The sealed passport application envelope will be handed back to you for expedited processing and that completes the Passport Execution Process. This envelope must be submitted to the US Department of State Passport Agency within 5 business days or it becomes invalid so ensure to overnight ship US Visa Connection right away.
