Special requirements for children under age 14 minor children must apply in person

 

Requirements for Children Under Age 14:

 - Both parents must appear at the Passport Acceptance Facility to sign the application for the minor.

 - One parent appears, signs, and submits second parent's notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 14 , authorizing passport issuance for the child or submits a notarized , written statement of consent from the non-appearing parent (including the child's name and date of birth, as well as parent's identification information or a copy of his/her ID) authorizing passport issuance for the child.

 - One parents appears, signs, and submits primary evidence of sole authority to apply (one of the following):

--Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent, or

-- Court order granting sole custody to the applying parent (unless child's travel is restricted by that order), or

-- Adoption decree (if applying parents is sole adopting parent), or

-- Court order specifically permitting applying parent's, or

-- Judicial declaration of incompetence of non-applying parent, or

-- Death certificate of non-applying parent.

1. Completed US Passport Application (DS-11).
Click the link, fill out and print the application.  Application may no longer be filled out by hand, but must instead be completed online, then printed. Also, do not sign this application until you are instructed to during Authorization process (step #9).

2. Proof of U.S. Citizenship (one of the following documents)

-A certified United States birth certificate

-An expired United States passport

-Original naturalization papers

3. Two identical passport photographs

4. Parent's identification issued by the government or state

-State issued driver's license (at least 6 months old) bearing a photo

-Military I. D. bearing a photo

-Government I. D. bearing a photo

5. Letter of Authorization

Download and complete the Authorization Letter.  

6. Order Form

Download and complete the Order Form.

7. Proof of Departure

A copy of airline ticket or travel itinerary.

8. Fees and Payment

Government Fees:

Make one money order to "US Department of State" for $120.00 (this is the government processing fee).

Our Service Fees by credit card or money order:

-24-48 hours - $150.00 + return shipping.

-5-10 day - 95.00 + return shipping

If you choose to pay by credit card, you may provide your credit card information on the order form and you may charge all fee's (including government fee).

9. Authorization

Bring all original materials to a Passport Acceptance Facility and notify the agent you are using an expediting service to "hand carry" your application. You will then sign the passport application form in the presence of the representative who will witness your signature and place the following items  in an envelope:

-Birth certificate

-Copy of parents drivers licenses

-Two passport-type photographs

-Application Form (Form DSP-11 and DS-3053)

-Check for 120.00 government fee made out to U.S. Dept. of State.

-Letter of Authorization.

-Copy of tickets or itinerary.

*Note: Please DO NOT seal the Order form in the postal envelope!

The representative will then seal the envelope which should be marked "To be Opened by U.S. Passport Office Only". Do not open this sealed envelope or the application will be invalid.  : Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".

10. Ship Your Package Using Fed-ex. Do not use U.S. Post Office Express Mail !!!
