Passport Information: Adding Additional Passport Pages

Note: If the visa pages in your passport are full, then you can request the addition of passport pages. Keep in mind that the pages in rear of the passport marked "amendments and endorsements" are not intended for visa or entry stamps. Step 1: Complete and Submit Form DS-4085 Application for New or First Time Passport (Complete the Online Passport Form DS-4085, then click CREATE FORM button) If for any reason you cannot complete the online Passport application, print the DS-4085 Passport Handfill Application and fill out by hand. Step 2: Complete & Submit Two Letters of Passport Authorization Print & Complete Two original copies of the Passport Authorization Letter. This form allows us to act as an agent on your behalf. One original goes to the Acceptance Agent and one original goes on the outside of the sealed envelope. Step 3: Complete & Submit One Passport Service Order Form Print and complete the Passport Order Form. Step 4: Submit Proof of Travel Provide an International ticket, e-ticket, itinerary or hotel confirmation for those driving. If traveling on business and no itinerary is available, a letter, signed by someone at the company, on company letterhead, which indicates the dates, location, name of person traveling and purpose of travel will suffice. (Note: You do not have to provide proof of travel if doing 5-10 business day processing) Step 5: Passport Processing Fees Government Fees: Make one check or money order to "US Department of State" for $142.00 (this is the government passport renewal processing fee).
